Wyot One Probation Officer makes a mistake. Inspectors, Union leaders and now Sadiq Khan say the responsibility lies with Gov because of cuts, offices running at 60% staff capacity, "impossible workloads".

Gov response: we are addressing the issue by sending all Probation Officers in the country on a training course.
