Green Man Sorry posted in the Tipsheet by accident.
Green Man No one is too blame here. It's tragic event. Boats can cause riptides,wash from boats move the pressure and currents can change direction in a second.

The Skipper has no doubt done the journey hundreds of times without incidents.
