Contaminated blood products were administered, leading to at least 2,000 deaths; these had been sourced from the US where some of the blood used has been bought from prisoners - who, seemingly, had not been tested appropriately.
Painkiling opiates are to be the subject of the new inquiry - not the first, by any means. It would appear that thousands were prescribed and given dosage levels that were far too high, resulting again in numerous unnecessary deaths.
Not a good day for the NHS...
