Several have asked to whom was this sent; the answer - the IOPC; Independent Office for Police Conduct. A slight problem may occur as part of the complaint is about Michael Lockwood, an accountant who is Director General of the IOPC. However if this body finds it concerning that police may have caused a death, I'm sure that won't stop them.
